The questionnaire below will ask questions about your identity. Please fill out the following questions in both forms to the best of your ability. We understand the information you provide may change as time goes on. You are welcome to supplement the information you provide at any time. Any questions you are unable to answer, please respond with N/A.

Submission of any information does not create an attorney-client relationship; Our Children’s Trust will only establish an attorney-client relationship through a signed retainer agreement.

Identity Questions:
Our Children’s Trust strives to have a diverse group of young plaintiffs with a diverse set of identities. All of your responses will be kept confidential and no information given here will preclude you from being part of the case.

1) What is your full legal name?

2) What is your gender identity? Gender identities can include female, male, non-binary, or many other identities.

3) What is your race and/or ethnicity? Examples include Black, Indigenous, Asian, Latinx, etc.

4) Are you affiliated with any tribe or indigenous group?

5) Are you religious or spiritual? If so, what are your religion or spiritual practices?

6) Do you identify as 2SLGBTQIA+ (two-spirit, lesbian, gay, bisexual, transgender, queer, intersex, asexual)?

7) Do you have a disability or learning difference? Some examples include loss of limb, limited vision or hearing, depression, bipolar, or other mood disorders, dyslexia, and ADHD.
